HOW DO YOU GET A Laminated Covid-19 Immunization record?
Purchase it online and we will mail it to the given address(es). If you want to purchase this product in person at our registry, you’ll save the $1 shipping fee (only $3/laminated card when picking it up/paying in person).

You can order multiple cards for you and/or members of your family and/or friends as long as you can provide the following information FOR EACH person requiring an Immunization Card:
- Alberta Health Care Number
- Date of Birth
- One of the months the person received one of their vaccinations (ie: APRIL)
- Full Mailing address including postal code
IMPORTANT: if a person was vaccinated outside of Alberta, they would need to first contact Alberta Health Services so that their Alberta Health records can get updated and this process takes a couple of weeks.

Please pay for the service and email or fax us (403.246.0647) the above documentation along with your payment receipt number. Once we receive the above information, we will print, laminate, and mail the card(s) to the address(es) you provided.
